Survey
The TIC results are used by the Tier 1/Universal Team to adapt action plans to PBIS needs by monitoring the implementation and maintenance of PBIS systems at the beginnig of the school year. TIC results help determine the most effective allocation of faculty, financial and other school resources. Teams meet together and take the TIC and enter one online.
- School-wide Evaluation Tool (SET)
Frequency: twice during a school's first year of implementation (pre and post); 20% of schools randomly selected in subsequent years
Timing: varies
Minimal score for fidelity: 80% on Expectations Taught and 80% overall
Who: District Team comes to school and interviews principal, students, and staff members
Survey
The (SET) is a research-validated instrument that is designed to assess and evaluate the critical features of school-wide effective behavior support across an academic school year. Information gathered can be used to a) assess features that are in place, b) determine annual goals, c) evaluate ongoing efforts, d) design and revise procedures and e) compare year-to-year efforts in the area of SWPBS.
- Self-Assessment Survey (SAS)
Frequency: once per year
Timing: near end of first semester (December)
Minimal score for fidelity: 80
Who: all staff members
Survey
The SAS is completed by all staff members to gage all staff members' view on the school's PBIS tier 1 implementation. Survey is taken online by every staff member anonymously.
- Benchmarks of Quality (BoQ)
Frequency: once per year
Timing: near end of the year (April)
Minimal score needed for fidelity: 70
Who: all members of PBIS Tier 1 Universal Team
The BoQ consists of three documents: the Coach Scoring Form, the Scoring Guide (rubric), and the Team Member Rating Form. The BoQ has a total possible score of 100. This score is derived from the 3-8 items in each of the 10 subscales; (1) Teaming (2) Faculty (3) Commitment (4) Data Analysis (5) Expectations (6) Recognitions (7) Lesson Plans (8) Implementation Plan (9) Crisis Plan (10) Evaluation. Each team member should go through the BoQ individually and score the Team Member Rating Form (can reference the Scoring Guide rubric). The team should then meet together and complete one BoQ using the Coach Scoring Guide. One BoQ is then entered online.
- Brief Attendance Systems and Procedures (bASP)
Frequency: once a year
Timing: early in the year (September/ October)
Minimal score needed for fidelity: 70
Who: PBIS Tier 1 Team
Survey is taken annually by staff members who work around attendance systems in the school. Teams submit one survey online.

- RtI All Staff Perception Survey
Frequency: once a year
Timing: towards beginning of year (October)
Who: all classroom teachers, special education teachers and administrators
Survey is taken online by staff members anonymously to gage staff members' view of the school's implementation of the RtI Framework around academics.
- School Implementation Review (SIR)
Frequency: once a year
Timing: towards end of the year (April)
Minimal score needed for fidelity: 70
Who: Building Intervention Team (Academic focused)
The SIR is a self-assessment instrument built around Wisconsin DPI’s vision of RtI. The SIR contains elements of a schools Universal (Tier 1) and Selected/Intensive (Tier 2/3) framework. SIR results are broken down into Universal Instruction, Selected/ Intensive Instruction, Balanced Assessment Universal, Balanced Assessment Selected/ Intensive, Collaboration Universal, Collaboration Selected/ Intensive, Leadership, Family Engagement, and Culturally Responsive Practices. The SIR should be taken by all members of your Building Intervention Team and an administrator online.
