The Student Immunization Law requires that all students through grade 12 must meet minimum number of required immunizations prior to school entrance. These requirements can be waived only for health, religious or personal conviction reasons.
According to our records, your child is out of compliance because either an up-to-date immunization record is not on file at the school or one or more immunization(s) are still needed (see reason for noncompliance marked below). To remain compliant with the law, please provide documentation from your doctor and/or health department with the month, day and year that your child received the required immunizations(s) or, in the alternative, select one of the waiver options prior to October 13, 2016 and return the form to your child’s school. Under Wisconsin state law and a Milwaukee County Circuit Court order, the Milwaukee County District Attorney’s office is responsible for enforcement of the immunization law including penalties for failure to immunize your child which may include:
- Exclusion from school for students in K5 through Grade 5, from 10/17/2016 through 10/21/2016.
- Court action and/or forfeiture penalty, for which you will have the right to come to court and request protection from the school’s decision to release your student’s immunization records to the Milwaukee District Attorney.
